Kill the monkey, jump into the water, swim E, resurface where possible and get out of the water to avoid three crocs. Kill them, grab the crack on the NE, shimmy right, fall onto the top of a block, drop off ledge onto a rock beneath you, do the running jump with grabbing a ledge of the rock on the E, then jump onto the next rocks on the N to reach the exit. Go through it, sprint to the other side of the room to avoid as many darts as possible, get to the room with the floor raised a bit, jump into the water, collect a Token on your right, resurface in the other air pocket and get out of the water. Three cannibals await you here. Kill them and notice the two rooms (probably the cannibals' huts). There is nothing in the right hut, so enter the left one, find a slot for the Token and use it to open the door located in the area between the two huts. Actually, there's no need to use the Token - the door can be pushed open by Lara if you stand in front of them and press Action. Independently of the way you open the door, get behind it and see your quest item waiting for you to collect it from the pedestal. Unfortunately, it can't be picked up, and a raising block in front of it can't be raised. This is the end of the adventure, so either kill Lara or pause the game and exit to title screen manually.
